He was honoured by the Hammers before their EPL clash with Crystal Palace after an outpouring of grief.
The funeral will held on May 5 at 2pm at Pinnaroo Valley Memorial Park, West Crematorium Chapel, Whitfords Ave.
The wake will be held at Crown Perth Astral Rooms, Great Eastern Highway, Burswood.
